Kippah Order Form

Please provide the information below.  This is a custom order so please allow 5-7 days for completion.  I will e-mail you when I receive the order and ask for confirmation before I begin.  I will advise when payment has been made and let you know when the order will be shipped.  If you need the kippah sooner, let me know and I'll do my very best to expedite completion.

Please note I do not answer e-mail on Shabbat (Sundown Friday evening until Sundown on Saturday evening.)
